At approximately 6:30 p.m. on May 7, 2020, an employee was using a set of 5-foot high job-made wooden scaffolds, standing on a 2x10 plank, while nailing a floor joist overhead. He lost his balance and fell to the concrete floor, hitting his head. He experienced dizziness and was hospitalized for a concussion.
