Incident Narrative
An employee was using his gloved fingers to clean out a gunite machine's feed wheel area when his glove was caught in a pinch point. The feed wheel then caught his left index and ring fingers, resulting in the medical amputation of his index finger and requiring surgery for his ring finger.
