Incident Narrative

On April 25, 2020, at approximately 2:00 a.m., an employee was being trained to use an electric pallet jack (PIT). While going in reverse, he lost control of the electric pallet jack and let go of the arm. As he let go of the arm, he stepped off the jack and his left leg was pinned against a rack. He suffered a lower left leg fracture.
