Incident Narrative

An employee was emptying a tip dumpster into a larger roll-away dumpster with a forklift. When the employee released the lever allowing the tip dumpster's contents to transfer to the larger dumpster, the weight shifted and the employee's left ring fingertip was caught between the tip dumpster and the forklift fork, resulting in a fingertip amputation.
