Incident Narrative
An employee was cleaning a roll former's tooling with a rag and manually jogged the machine with a remote control. The employee's glove and cleaning rag came in contact with the roll former, pulling his finger between the rolls. His left index finger was pinched and rolled over, resulting in surgical amputation of the finger to the first knuckle.
