Incident Narrative
An employee was driving a forklift in a warehouse. As he was backing up with a skid (wood pallet) load, the load caught on a beam post; his head hit the handle of the forklift and his abdomen hit the steering wheel. He suffered a vertical cut between his eyebrows, about 2-2.5 inches long, as well as an abdominal injury with possible internal bleeding.
