Incident Narrative

An employee was working from a 15-foot-high scissor lift, replacing lightbulbs along a ceiling. An overhead crane struck the scissor lift, causing it to tip over. Its basket fell onto a table on the floor, then ejected the employee; the employee suffered a broken elbow, a possibly broken arm, a broken collarbone, and a head injury.
