An employee was operating a roll threader machine to manufacture a metal bolt. The employee picked up a piece of material and positioned it into the threader dies to thread. He closed the dies with the foot pedal and used his left hand to help balance the material. When the material began to spin, his glove was caught and his left arm was twisted and broken.
