Incident Narrative

An employee was feeding bar stock into a bar straightener. The employee's apron became wrapped on the spinning bar; as the employee tried to free the apron, his left hand was caught between the bar and the machine guard. The tip of his little finger was amputated and the middle and ring fingers were broken.
