Incident Narrative

An employee was pushing a steel mud bucket inside a tunnel work area. The bucket was loaded with pieces of lumber and C-shaped steel beam rings. A piece of the steel caught the edge of the tunnel wall, moved back, and caught the employee's right index finger against the upper edge of the bucket. The employee suffered an amputation to the finger.
