Incident Narrative
An employee was dismantling a scaffold while working on an elevated platform that was lifted on the forks of a telehandler forklift and was equipped with a steel guardrail system. The employee was working from one side of the platform and slid a 24-foot aluminum scaffold platform from the wall bracket onto the top of the guardrail. The scaffold platform pivoted on the guardrail and fell 12 feet to the ground below. The employee was holding onto the platform as it fell and was ejected from the platform onto the ground below. The employee was hospitalized for a concussion and fractures to the hip, wrist and ankle.
