Incident Narrative
An employee was using a tube squaring machine to cut tubes. The employee had turned off the machine in order to tighten the clamping shell. The machine had not come to a full stop as the employee moved his right hand into position to turn the tensioning screw. While turning the tensioning screw, the employee s right hand slipped and his right ring finger contacted the square tube blade resulting in a fingertip amputation without bone loss.
