Incident Narrative

An employee was operating a forklift with a mule attachment. The metal frame of the mule came off the metal bracket on the forklift. As the employee attempted to adjust it, his pinky finger was pinched between the metal bracket on the forklift and the metal mule frame. The top one-third of the employee's pinky finger was amputated.
