Incident Narrative
On February 4, 2020, an employee was coming down off the rooftop of a building after checking on a humidifier issue on an air handler. The employee climbed down from the roof via a fixed ladder. In order to close the roof hatch, the lock must slide out. When the employee slid the lock, the hatch was caught by the wind and came down. The employee's finger was caught between the hatch and the frame, resulting in an open fracture of the left index finger/partial amputation.
