Incident Narrative

Employees were using an extending boom forklift to move a dumpster. The injured employee was securing the dumpster to the forklift forks with chains. The employee's foot was leveraged against the side of a railing when the dumpster and forklift shifted slightly and pinched the employee's toes between the dumpster and the railing. As a result, two of the employee's right toes were amputated.
