Incident Narrative

An employee was moving a pallet of material using a reach truck/powered industrial vehicle when the pallet began falling from the forks. The employee reached out to stop the materials from falling and then dismounted the truck to stop it. The truck continued to move, and his left leg was pinned and crushed between the truck and a nearby railing.
