Incident Narrative

An employee was working from a scissor lift and wiring a light fixture near a roll-up door when the door was activated and the employee became rolled up in the door. Nearby employees activated the door to release the employee, who fell approximately 10 feet to the concrete floor and suffered broken ribs.
