Incident Narrative
An employee was replacing chains on a forklift, whose mast was hooked and supported by an overhead 5-ton crane. When the support nuts at the bottom of the fork chains were removed, the forklift forks and backstop fell on the employee, knocking him down and pinning him to the floor. He suffered injuries to the left hand, neck, back, and collarbone and a broken left femur. During surgery, his left ring finger was amputated to the first joint.
