Incident Narrative

On January 10, 2020, an employee finished drinking a bottle of sports drink and set it down empty in a maintenance shop before leaving for the weekend. The bottle was then used to hold brake cleaner. On January 3, the employee refilled the bottle with water, began drinking, and became nauseated and vomited after the first few gulps. The employee was hospitalized.
