Incident Narrative

An employee was attempting to reposition a ride-along pallet jack between steel racking units in a freezer when the pallet jack hit an upright. The employee's hand was outside the steering guard at the time and was caught between the steel upright and the frame of the pallet jack. As a result, the employee's left pinky finger was amputated.
