Incident Narrative

On January 8, 2020, at approximately 3:15 a.m., an employee was setting up a print press when the employee attempted to clean the print rollers. The employee's glove was pulled into the machine's rollers, crushing the employee's right hand. The employee sustained four broken fingers and the amputation of the pointer, middle, and index fingertips. The machine was running at the time of the incident.
