Incident Narrative

An employee used a sit-down forklift truck to load a 4 ft. x 4 ft. x 4 ft. tote filled with scrap onto a flatbed truck. The flatbed truck was backed up to the dock and was slightly lower than the dock. As the forklift was being driven down the dock plate, the flatbed truck rolled forward about 7 or 8 feet. The forklift fell and hit the ground in the upright position. The employee jumped out the left side of the forklift, landed on his left side, and rolled onto his back side, suffering multiple injuries, including a fractured left hip, shoulder injuries, a left elbow injury, and a head injury.
