Incident Narrative

On January 3, 2020, a press operator was using the press machine on the draft gear line and a welder was observing the press operator insert pins into a draft gear. The pins were not correctly inserted into the draft gear and the welder attempted to insert the pins properly. The press machine cycled, pressing down on the draft gear, and the welder's hands were pinched between the pins and conveyor guide rail, resulting in amputation of the left index fingertip and a laceration to the right pinky finger.
