Incident Narrative

At 6:00 a.m. on December 20, 2019, an employee was positioning a hoist to move a rack of tooling to a wash bay. As the employee operated the hoist to remove the slack from the chain, the eyehole of the hook pinched and smashed the employee's left index finger due to the weight of the rack being lifted. The employee's fingertip was amputated.
