Incident Narrative

An employee was installing a piece of equipment in the facility that involved moving the equipment into place and plugging it into a receptacle. The employee realized the plug and the receptacle did not match. When the employee removed the receptacle from the wall to inspect the wiring an arc flash occurred and burned the employee's hands.
