Incident Narrative
An employee was placing a 425-pound turbine disk into a crate for shipment. While attempting to position the turbine disk, the employee's right middle finger was caught in between a 2x4 inside the crate and the turbine disk. When the employee attempted to pull out the right middle finger, the right distal phalangeal tip was amputated.
