Incident Narrative

An employee was helping another employee investigate the operation of a lift gate on a box truck. The second employee lowered the lift gate, but the mechanism was stuck. The second employee freed the mechanism and used the controls to lower the lift gate. The lift gate ramp then needed to be flipped over. Both employees grabbed the ramp plate to flip it over and as they were bringing the ramp plate over, it fell on the first employee's right foot. The first employee was hospitalized with four toe fractures.
