Incident Narrative

An employee was operating a powered industrial truck (PIT) when the PIT struck a sprinkler head, creating a flood of water from overhead. The employee disembarked the PIT and slipped and fell on the wet concrete floor while attempting to exit the flooded area, suffering a serious spinal injury.
