Incident Narrative

An employee was guiding a steel structure onto anchor bolts for securement. The steel structure began to rotate and the employee grabbed the base of the structure to prevent movement. The employee's finger was pinched between the steel structure and the anchor bolts resulting in an amputation to the left middle fingertip.
