Incident Narrative
An employee was involved in lifting a pipe out of a double service well rig and taking apart the slip bowl assembly. The operator had attached the lift sub to bring the pipe up when the threads failed, causing the pipe to come back down and amputate four of the fingers on the employee's left hand.
