Incident Narrative

An employee was checking the material level in the melter on a striping truck, which was parallel to a supply truck from which a hose was running into the melter. The hose ruptured and the employee was sprayed with molten thermoplastic. He suffered second- and third-degree burns to the face and the back of the neck, and some plastic adhered to his skin.
