Incident Narrative

An employee was finished performing duties on the warehouse floor. While reaching down to grab his laptop bag, he had positioned his hand on a fusion table. A nearby forklift carrying a PVC pipe was rocked; the pipe went to the back of the forks and the edge of the table. The employee was caught and sustained a revision amputation of the left pinky finger.
