Incident Narrative

On October 30, 2019, an employee was preparing an area to throw bundles of mail underneath the automated parcel bundle sorter. He got on a powered operated pallet jack to move it. While he was moving the pallet jack, the control handle unexpectedly came up, causing the pallet jack to come to a quick stop. He fell off of the pallet jack and landed on his back on the concrete floor. He was hospitalized with a fractured back.
