Incident Narrative

An employee was changing a roll of paper on a rack with a 10- to 12-foot bar. The bar's pin was removed on one end. When the employee pulled up on the end of the bar, it came loose, causing the opposite end of the bar to fall. This then caused the entire bar to fall, and the employee's left fourth finger was caught between the bar and the rack, amputating the fingertip.
