Incident Narrative
An employee was loading dump trucks with an excavator when the gate on a dump truck would not latch. The employee got off the excavator and tried to manually latch the gate shut. At the same time, the dump truck driver was trying to latch the gate with the hydraulic system. The hydraulic system caused the gate to latch and crush the employee's fingertip.
