Incident Narrative
An employee was taking a lift truck outside to perform preventive maintenance on it. While he was moving the lift truck, he heard a squeaking noise coming from the engine compartment. He opened the bonnet/hood and was trying to figure out where the noise was actually coming from. The engine was running at the time. While he was trying to get a better look at the water pump area, his right index finger was caught between the fan belt and pulley, resulting in a finger amputation.
