Incident Narrative

After installing rooftop HVAC systems, an employee started to descend the extension ladder from the roof when it retracted, causing the employee to fall approximately 25 feet to the ground. The employee suffered four cracked ribs, a fractured pelvis, and a fractured vertebra.
