Incident Narrative

Three employees were installing a cage back into the crusher. They were transporting the cage back to the crusher utilizing a trolley system with three-ton chain falls attached to the trolley. The cage, that weighed 8000 lbs., tilted catching the injured employee's left hand between the chain and the striker plates resulting in amputation of the middle finger just above the second knuckle. The index finger was crushed.
