Incident Narrative

At about 2:30 p.m. on October 7, 2019, an employee was trying to retrieve a crushed cup from a rotary filler. The machine automatically cycled, and the employee's right middle finger was caught in the head heater area, causing an amputation between the first and second joints. The heater head was unguarded at the time of the incident.
