Incident Narrative
An employee was sealing the roof vent on a grain bin (about 24 feet above the ground) for fumigation. The weld holding the employee's fall protection anchor point failed; the employee fell from the roof of the bin, suffering broken bones in the ankle, heel, and pelvis on the left side.
