Incident Narrative

On September 18, 2019, at approximately 20:30 hours, an employee was replacing three fluorescent light units with LED light units in the HR office. The lights were turned off using two toggle switches, but were still energized. While installing the second LED light unit, the employee sustained an electric shock, second degree burns to his right hand, and a labral tear and crushing injury to the right shoulder.
