Incident Narrative
An employee was standing on a dunnage rack to reach pizza items on the top shelf in the freezer area. When the employee reached for the items, a bag of cheese opened and cheese scattered onto the floor. The employee stepped backward off the rack and then slipped on the cheese that was on the floor. He fell against the racks that were behind him and sustained a punctured lung, broken shoulder, and two broken ribs. He was hospitalized.
