Incident Narrative
An employee was welding and grinding on a 14-inch, 40-foot pipe resting on two sawhorses. As employees adjusted the pipe, it fell off the sawhorses and struck the employee doing the welding, who suffered a broken right thigh bone, a broken left ankle, and an amputated fingertip on the right hand.
