Incident Narrative
An employee stepped up onto the right side of a trailer's tire bar, which is 24.5 inches high, to set a deck pin. When the employee stepped back down, his left ankle twisted on the asphalt, causing him to fall on his back and strike the back of his head on the asphalt. He suffered a concussion, a laceration to the back of his head, and a sprained left ankle.
