Incident Narrative

An employee was preparing to clean up grain from the outside of a grain bin using a grain vacuum. When the vacuum was started, it sucked on to the grain door. It then released suddenly as the employee pulled it off; the employee fell backwards and hit his head and back on a drag conveyor. He suffered a contusion to the back of the head.
