Incident Narrative

An employee was operating a pallet jack to unload pallets from a semi trailer. The stack of pallets would not clear the door of the trailer, and the top pallet became stuck on the trailer door, pulling it closed on the employee's arm. The employee suffered a broken left forearm and required surgery.
