Incident Narrative
An employee was operating a telehandler forklift while wearing his personal fall protection harness. When he went to back up, a lanyard that was hanging down and not attached to the harness caught under the front wheel and jerked him violently in his seat. He was wearing his seat belt at the time. He struck the metal guards on the side seat of the forklift and fractured vertebrae in his back.
