Incident Narrative

An employee was climbing a ladder to trim a palm tree. Before reaching the top, to tie the ladder to the palm tree, the employee fell backwards off the ladder. He landed on the brick patio 10 feet below and sustained fractured vertebrae in the neck and back and fractured ribs.
