Incident Narrative
A 6-inch riser for sprinklers was being installed. To attach the riser to the system, an employee pulled off a 6-inch grooved cap. The system discharged air, which blew the coupling and cap into the employee's left thumb. The thumb was amputated and the employee was hospitalized.
