Incident Narrative

Employees were core drilling into the foundation of a new building. When they finished drilling, they tried to pull the drill bit out of the machine using two wrenches positioned on either side of the drill bit. An employee was attempting to loosen the bit when the wrench came loose and struck the employee's right eye, requiring hospitalization.
